Office Policies and Information:

  1. Please bring any documents showing your exact diagnosis to your first appointment

  2. Documentation of standard-of-care treatment for the qualifying condition must demonstrate the failure of conventional medications to control currently active symptoms within the last 12 months.

  3. For those with a history of psychiatric disorders or personal history of addiction, including alcohol and substance abuse, continuity of care by your mental and behavioral health specialist must be established within the last 12 months.

  4. All major credit cards and Apple Pay are accepted.  Fees for each session must be paid at the time of visit.
